The gymnast waiver form can be downloaded in Microsoft Word (.doc) format or in Adobe Acrobat (.pdf) format. Please follow all instructions specified on the form.
Entry deadline is MARCH 12, 2007 for qualified gymnasts.
Return completed team registration form, waiver forms, and one check for all entry fees to:
